GENERAL RESPONSIBILITIES
The Health Care Coordinator Supervisor provides direct supervision to the Healthcare Coordinator team. This role works to ensure that the team provides real-time, team-based care navigation, coordination, and communication between the healthcare organization and the patient. Partners with operations and provider leaders to maximize patient access to care. Collaborate with site management teams for daily operational needs.
MINIMUM QUALIFICATIONS
- Bachelor's degree or relevant training and experience in health care administration or, another related field.
- At least three years of demonstrated direct supervisory experience.
- Strong conceptualization capabilities to introduce new ideas and solutions that will improve methods, procedures, or systems.
- Minimum 3-5 years of experience in referral coordination and/or health information systems. Demonstrated knowledge of medical terminology and abbreviations along with an understanding of patient flow processes and scheduling
- Ability to appropriately communicate with customer service escalations and able to work under stressful conditions.
- Knowledge of and ability to apply established professional customer service concepts, principles and practices.
- Proficient computer skills and experience working with electronic medical records and internet portals.
PREFERRED QUALIFICATIONS
- Graduate of a medical-related certification or training program.
- Two or more years in a medical community health clinic setting.
