A Community Health Worker works closely with medical providers and social services agencies to provide short-term care coordination and connection to resources.  Additionally, helps support clients in order to improve their health and general well-being through education.

Minimum Qualifications:

• Some clinical experience (CNA, MA, phlebotomy  or other related position) 
• Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
• Knowledge of community agencies and resources.
• Ability to plan, implement and evaluate individual client care plans.
• Ability to communicate medical information to health care professionals and care coordinators over the telephone.
• Skill in use of personal computers and related software applications, including email.

Preferred Qualifications:

• Bilingual in English/Spanish
• Experience working with underserved and/or aging populations.
• 2 years experience working with Medicare and major insurance carrier regulations, procedures and benefit plans.
• 5 years experience working with clinical coding systems.
• Experience using Electronic Health Record Systems, including scheduling.
