Case Manager - Allumbaugh House

GENERAL RESPONSIBILITIES       

Provide behavioral health treatment services for Allumbaugh House. Coordinates and completes treatment needs assessments, treatment plan updates, discharge planning and outpatient referrals. Will also perform the duties and responsibilities of a Psychiatric Technician when milieu/staffing needs determine.

 

ESSENTIAL DUTIES

  1. Provide on-site chemical dependency screening or assessment, and crisis intervention under the supervision of the CADC.
  2. Educate patients, families and community members about behavioral health disorders and treatment
  3. Complete treatment and discharge plans in cooperation with Allumbaugh House medical clinicians, counselors and nursing staff.
  4. Participate in case staffing and consultation with appropriate medical clinicians, counselors and nursing staff.
  5. Counsel patients in individual or group sessions to assist them in identifying recovery support and aftercare needs.
  6. Maintain collaborative linkages with other community programs and services.
  7. Monitor, evaluate and record patient progress with respect to treatment goals.
  8. Support patient participation in treatment plan activities (making follow-up appointments, arranging transportation, etc)
  9. Refer patient and/or family to community resources for continuing treatment, housing, and supportive services as indicated by the needs of the patient. Provide case management services including completing documentation that would assist client to access community resources (disability forms, client requested correspondence)
  10. Provide support and guidance to other staff members in discharge planning/case management related services
  11. Clearly document all patient activities related to case management
  12. Comply with all current TR policies, including HIPAA policies.
  13. Attend and participate in all staff meetings.
  14. Portray professionalism to clients, other TR department personnel, referral sources, and the community during daily interactions and appearances.
  15. Utilize training funds appropriated for educational advancement consistent with CEU policy; attend required workshops as they occur.

MINIMUM QUALIFICATIONS

  • Successful clearance of Department of Health and Welfare criminal background check.
  • Bachelors Degree in Social Work or related field.
  • Two years work experience in the field of alcoholism, substance use disorders and mental health.
  • Working knowledge of Boise aera social service resources.
  • One year work experience in a clinical mental health setting.
  • Good written and verbal communication skills.
  • Experience working with disenfranchised populations.

PREFERRED QUALIFICATIONS

  • Two or more years experience in the field of alcoholism, substance use disorders and mental health with case management experience
  • Experience working in a medical setting, crisis mental health and/or detoxification setting, or as a member of a multidisciplinary team.
  • Knowledge of computer and email systems, as well as EMR.
  • Knowledge of community resources and Patient Assistance/Medication Assistance Programs.
  • Familiarity with State of Idaho Health & Welfare Mental Health system.
