Case Manager

GENERAL RESPONSIBILITIES       

As part of a multi-disciplinary provider team, coordinates and provides needs assessments, social service referrals, crisis intervention, case management, client advocacy and related services for patients receiving medication assisted treatment (MAT), as well as those from the general clinic population, as appropriate.

 

ESSENTIAL DUTIES

  • Act as the main point of contact for potential and existing MAT patients.
  • Triage care with medical and behavioral health provider(s).
  • Maintain case management caseload of MAT patients.
  • Maintain MAT registries, data bases and pharmacy portals as necessary.
  • Coordinate services with other community agencies involved in the treatment of MAT patients.
  • Document all patient encounters, or work done on behalf of patient, in the medical chart.
  • Complete encounter billing documentation for all work done on behalf of clients.

General Case Management Duties

  • Maintain collaborative linkages with other programs that serve the patients. Linkages will include existing health care providers, local social services agencies, shelter providers, housing services, and other appropriate providers.
  • Assist patients in the completion of paperwork associated with their care, including but limited to: Social Security, Medicaid, Medicare, Patient Assistance Program, and external specialty programs.
  • Communicate with patient financial services and accounting staff as needed to coordinate accurate reporting for MAT patients receiving services under specific grant programs. This communication may consist of staffing individual patient accounts or clarifying general situations and issues.
  • Provide general case management services, including information and referral, advocacy and brokering to TR Boise and Nampa Clinic patients.
  • Provide case management services to patients who are unable to keep scheduled appointments. Identify and help eliminate barriers to increase patient consistency and compliance with treatment.
  • Provide crisis intervention to patients as needed.

MINIMUM QUALIFICATIONS

  • Bachelors Degree in Social Work, or undergraduate degree in a related field.
  • Licensed, or license-eligible, in the State of Idaho (if relevant for degree).
  • Two years' experience in chemical dependency or substance use treatment program.
  • Clearance through the Idaho Department of Health and Welfare Criminal History background check.
  • Demonstrated willingness and desire to work with the homeless, low-income and substance misuse populations.
  • Demonstrate an understanding of the problems of the mentally ill and chemically dependent populations.
  • A working knowledge of local area social service resources for targeted populations (city, county, state and federal).

PREFERRED QUALIFICATIONS

  • Experience working with patients with addictions.
  • Experience working in a medical setting
  • Experience working within a multidisciplinary team.
  • Preferred bilingual skills (English/Spanish).
