Community Dental Health Coordinator

GENERAL RESPONSIBILITIES      

The primary role of the CDHC will be to reduce the oral-health disparities of patients in the Terry Reilly Health Services network by linking patients in need of oral health services with existing oral health providers for the purposes of reducing oral disease. The CDHC will be an integral member of the dental, patient navigation and population health teams, helping with integration and coordination of dental care. Critical areas of contact will include; clinical assessments and brief interventions of care, examining and resolving social determinants of health that may impact patient engagement in their care. Patient and staff education, and community outreach. The CDHC will encourage patients to complete treatment, coordinate transportation, educate the population about prevention, refer patients to supplemental nutrition programs and will navigate Medicaid and other systems of care. Process Improvement efforts will also be an important role of the CDHC and will include; phone calls and chart review for data gathering, working in reporting systems with timely submission of outcomes data. The CDHC must be competent in communication, facilitation, coordination, collaboration, measuring, monitoring and communicating outcomes and documentation of activity.

MINIMUM QUALIFICATIONS

• Fulfills requirements of Dental Hygienist job description
• Certified Community Dental Health Coordinator (CDHC)
• Licensed Registered Dental Hygienist
• Proficient in Microsoft Office products
• Proficient in Electronic Health record

PREFERRED QUALIFICATIONS

• Clinical experience in a community health or group practice setting.
• Bilingual English/Spanish
