Community Health Worker Program Coordinator

GENERAL RESPONSIBILITIES       

Under indirect supervision, works closely with medical providers, primary care teams, and social services agencies to provide short-term care coordination and connection to resources in supporting clients to improve their health and general well-being through education and provision of coordination of care and services. Implements off-site/on-site outreach programs focusing on health promotion, preventive, curative and case management. Responsible for developing, implementing and managing Promotores program to address social determinants of health in low-income, Latino communities such as migrant seasonal farmworkers (MSFW).

Essential Duties:

  • Assists in providing access to needed services and builds individual, community, and system capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, patient navigation and follow-up, community health education and information, informal counseling, social support, and advocacy.
  • Build and maintain a strong with relationship with the Terry Reilly care team and collaborative partners to ensure that our outreach efforts increase access to healthcare services and patients are receiving the level of follow-up as identified in outreach registry.    
  • Develop a referral and follow-up process for patients seen at outreach events and mobile health clinics. Provide care coordination and follow-up where needed including documentation in registries and reporting. Documents all client encounters and contacts made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes achieved by client in an effective manner.
  • Develop, Implement and Manage Promotores program within the MSFW communities. Identify and supervise at least 2 Promotora's whose focus will be to act as a liaison to assist and educate patients and their families of community resources that will lead to an improved quality of life and better personal health within their communities.
  • Assess the need for and implement patient education programs for MSFW adults and children, especially with regard to diabetes, cardiovascular disease, hypertension, cancer, and obesity.
  • Provide basic case management as needed to patients living in labor camps, as well as other MSFW and general population patients as referred by providers. Case management includes but not limited to patient education, establishing self-management goals, referrals services, and collaboration with clinical, pharmacy, nursing, and social service staff.
  • Develop and distribute brochures, posters, and other promotional materials to inform farmworkers about primary health services at Terry Reilly as well as other outreach events.
  • Plan and/or participate in at least one health fair a month at or near each farm labor housing development during growing season; collaborate with SWDH, local diabetes and heart association affiliates, BSU, pharmacies, USDA, etc. Participate in other health fairs targeting both farmworkers, and the general public, as appropriate.
  • Network with appropriate agencies (such as Migrant Head Start, Idaho Migrant Council, and Idaho Legal Services Association) and inform them of Terry Reilly services, as well as receiving information about their services. Keep Terry Reilly staff properly informed regarding services available from such agencies.
  • Build and maintain a positive relationship with the local schools to facilitate referrals to our clinics.
  • Provide translation within the clinics, while at outreach events, for the Terry Reilly website, and for promotional materials, as needed.
  • Assess need for new services and outreach methods to maximize farmworker outreach by TR; assist with developing resources for any new projects.
  • Continuously expands knowledge and understanding of community resources and services. Facilitates client access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services. Assists clients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.
  • Facilitates communication and coordinate services between providers and the clients/patients.
  • Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between clients and institutions.
  • Travels extensively to client homes, community locations, various agencies, and other outreach destinations.
  • Supports Outreach and Communications team at outreach events in the community, including enrollment events specific to Medicare.

MINIMUM QUALIFICATIONS

  • Bachelor's Degree in health promotion or other health related field, education, communication, or social services.
  • Bilingual/biliterate (English/Spanish).
  • Comfortable with and successful at public speaking.
  • Culturally competent and interested in working with low-income Hispanic population.
  • Physical ability to do home visits.
  • Available for regular weekend and evening duties.
  • Must have reliable transportation to get to different outreach sites.
  • Knowledge of community agencies and resources.
  • Demonstrated ability and experience to do project management.
  • Ability to work independently and use sound judgment.
  • Well-organized, creative, flexible, open-minded and able to manage several tasks simultaneously.
  • Ability to establish and maintain effective working relations with public and other staff persons. Strong customer relations skills.
  • Ability to handle in a professional manner demanding or difficult patients and clients.
