Healthcare Coordinator - Canyon Dental / Caldwell

GENERAL RESPONSIBILITIES       

The Healthcare Coordinator provides real-time, team-based care navigation, coordination, and communication between the healthcare organization and the patient. The Healthcare Coordinator receives, processes, schedules and completes the tracking protocols on all medical referral requests from assigned care team clinicians. This includes all in-house and outside referrals for diagnostic testing, medical specialists, or other providers. This role will also coordinate care and collaborate with Terry Reilly team members and community healthcare partners to support higher standards of wellness care for our patients through the management of care registries. This position will organize and manage health information data ensuring its quality, accuracy, accessibility, and security in both paper and electronic systems.

ESSENTIAL DUTIES

  1. Receives and reviews for accuracy all referral requests/orders from care team members, and then initiates and documents the referral tracking process per established Terry Reilly policy.
  2. Completes forms for Medicare, Department of Health, Social Services, Insurance and specialty offices as required. Completes Release of Information (ROI) as required.
  3. Maintains ongoing communication throughout the referral process with the care team/PCP through accurate and timely documentation of referral details/progress in the referral tracking document.
  4. Answers telephones, screens calls, takes messages and provides information concerning the referral process.
  5. Maintains confidentiality, security, and physical safety of patient and employee medical records in electronic or paper format.
  6. Maintains a predetermined organized record format, consistent with applicable law. Maintains the content and format of electronic/paper medical records in a uniform manner.
  7. Ensures the quality of medical records by verifying the completeness, accuracy, and proper entry into computer systems.
  8. Uses available computer applications to retrieve, document, assemble and analyze patient data for the purpose of improving patient care or controlling costs.

 

MINIMUM QUALIFICATIONS

  1. Minimum one year experience in the medical field with knowledge of medical terminology and abbreviations and understanding of patient flow processes.
  2. Ability to appropriately communicate/deal with irritated or frustrated clients; to work under stressful conditions.
  3. Knowledge of and ability to apply established professional customer service concepts, principals and practices.
  4. Ability to prioritize and execute a variety of tasks simultaneously.
  5. Ability to work with frequent interruptions.
  6. Proficient computer skills.
  7. Keen organizational skills.
  8. Ability to read, write and communicate clearly in English and/or Spanish.
