Outreach Specialist

Hours/Days: On Call

ESSENTIAL DUTIES

• Offer education and outreach throughout the community, i.e. food banks, farmer's markets, mobile health clinics, etc.
• Collaborating with community partners with health education initiatives,
• Assist patients with pre-registration and sliding fee applications while out in the community
• Schedule patient appointments during community events
• Offer light case-management during home visits
• Provide basic health screenings such as blood pressure and glucose checks
• Provide education on Medicare, Medicaid, and the Your Health Idaho Insurance Exchange
• Provide educational seminars on medical, dental, and behavioral health services
• Increase awareness and facilitate conversations regarding population health
• Attend and participate in appropriate meetings, functions, and seminars; assisting with projects and tasks relating to medical, dental and behavioral health
• Work well under pressure; meeting multiple and sometimes competing deadlines. The incumbent shall demonstrate cooperative behavior with colleagues and supervisors  
• Must be willing to travel within the Treasure Valley and may be required to work weekends and evenings
• Preform other duties as assigned

MINIMUM QUALIFICATIONS

• College Education in Health Science, Public Health, Pre-med, Nursing, etc
• Two Years' experience in health promotion

PREFERRED QUALIFICATIONS

• Bi-lingual in English and Spanish
