Registered Dietitian Nutritionist

GENERAL RESPONSIBILITIES     

To provide outpatient nutrition counseling and education services to patients with diabetes and prediabetes.

 ESSENTIAL DUTIES

1. Provide outpatient nutrition education and counseling services for patients with diabetes and prediabetes, as well as comorbid conditions.
2. Employ comprehensive case management approach, allowing for regular follow-up with patients/clients on diabetes self-management goals and interdisciplinary team interventions.
3. Collaborate with medical, pharmacy, social service, and other clinical staff on clinical outcomes and patient/client treatment interventions.
4. Establish and achieve quality clinical outcomes, including health metrics, patient encounters, treatment plan implementation, and interdisciplinary team interactions.
5. Perform administrative tasks, e.g., scheduling patient appointments, documenting patient/client visits in EHR within 48 hours of the visit, and participating in clinical care team meetings.

PERFORMANCE CRITERIA

1. Develop/tailor and successfully administer screening tool to identify patients/clients most likely to benefit from chronic disease case management.
2. Create/modify and effectively implement self-management tool for nutrition and behavioral modification goal setting.
3. Foster positive working relationships and open communication with clinic staff.
4. Facilitate patient outcomes that yield improved quantitative and qualitative health measures.
5. Produce accurate and complete program outcome data.
6. Maintain confidentiality and comply with HIPAA policies at all times.

MINIMUM QUALIFICATIONS

1. Registered dietitian nutritionist with current Idaho State Board of Medicine Dietitian license.
2. Demonstrated experience in nutrition counseling and education.
3. Strong communication and organizational abilities.

 PREFERRED QUALIFICATIONS

1. Experience working with a wide variety of patient populations.
2. Bilingual (English, Spanish) a plus.
